Mr May collided with a car along the Ballarat-Daylesford Road last Monday night and was airlifted to The Alfred hospital in Melbourne.
He had spent the past week in a critical condition after sustaining serious injuries in the crash.
In a statement, Hepburn Shire mayor Sebastian Klein said Mr May had made a huge contribution to public life across the region which would not be forgotten.
“His contribution to municipal and community life over the decades of his life is unmistakable,” Cr Klein said.
“Among his colleagues here at our council and among our communities, he will be sorely, and sadly missed. To his family, we send our warmest thoughts and condolences. We share your grief.”
As well as his time as mayor, Mr May was well known as the Greens candidate for the seat of Ripon in the 2014 state election.
